Do I have to republish for every test?

Only once, for that one line. After that, variations go live without a Webflow publish.

Does it work with Webflow Ecommerce?

Yes. You report the purchase on the confirmation page, and add-to-cart or checkout start work as click goals.

Does it change my Webflow build?

No. The page only changes in the visitor’s browser. In the Designer everything stays as it was.
